Community Partner - New category this year! Nominate a community partner within your area that is a champion 
for hospice and/or palliative care. This could be an individual or an organization that goes above and beyond with 
your team to advance your meaningful mission.  

• Complete the Nomination Form 
• ONLY submit the Nomination Form; no additional documents will be considered.
• Previous award recipients are not eligible for nomination.
• Following the September 11th deadline, nominations will be vetted and then sent to an out-of-state judge for review/selection. One award recipient will be selected for each category.
• You will be notified of results by September 28, 2026.
• Recipients will be recognized and receive their award on October 16th, during the 2026 Hospice and Palliative Care Conference in Benton, AR.
